All forms and evidence should be emailed to england.nshcs.accreditation@nhs.net. Do not embed any documents into the form. We recommend that you attach a compressed (ZIP) file to the email.
We will assess the application form once we receive all the requested information. We may need to contact you for further information or clarification. The timescale for accreditation depends on how quickly we get all the information we require. Accreditation is granted for a fixed period of 5 years.
